A form in STQRY Engage is a set of questions people answer in their browser or in your app. You build forms in Manage Account → STQRY Engage → Forms, which needs the CRM Admin permission.
Create the form
- Go to Manage Account → STQRY Engage, then click Forms.
- Click Create form.
- Enter a Name. Only you and your team see the name; respondents see the Title.
- Add a Title and Instructions if you'd like, and choose the Primary language.
- Choose an Action (see below), then click Create form.

The Create form dialog. You can change everything here later.
The form opens in the editor. Its four tabs are Form details, Fields, Design and Notify, and the Preview on the right shows the form as respondents will see it. Click Save Changes to keep your edits.

Choose what happens to responses
The Action setting decides whether a response adds to your contacts.
| Action | What happens |
|---|---|
| Create people and marketing list subscriptions | A response with an email address adds or updates that person in People, and signs them up to any marketing list they ticked. This is the default. |
| Store submissions, do not create | Responses are kept with the form and nothing else changes. |
See How form submissions add people for exactly how a response is matched to a person.
Form details
On the Form details tab you can change the Title, the Instructions and the Submit button text. The instructions take formatted text, so you can add links and emphasis.
To translate the form, add a language tab and fill in the title, instructions, button text and field labels for that language.
Change how the form looks
On the Design tab, choose a Theme and a Header layout. The layouts are None, Image, Image & title, Short header and Tall header. The image layouts let you add a Cover image, and Image also takes a Logo image with its Logo position and size.
The form starts with your account's default theme.

Choose a theme and a header layout on the Design tab.
Archive a form
To retire a form, click its archive action in the Forms list. Archived forms are hidden from the list until you set the filter to Archived, where you can Unarchive them.
A form that's still used on a screen can't be archived. Remove it from the screen first.
Next steps
- Add fields to a form: add the questions.
- Get notified of new form responses: hear about each response by email.
- Share a form with a link or QR code: start collecting responses.
